Overview

The CNM2 FIT is a pivotal management role in a busy Frail intervention Team with responsibilities for case and staff management and patient flow through the Emergency Department and through the acute hospital setting with links out to the community.

The post-holder will provide an innovative, integrated service to those individuals who have been identified as frail or at risk of frailty. The role requires a caring, dedicated, reliable and patient - focused individual who enjoys meeting new people. The aim of the role is to support individuals to manage their health and wellbeing and continue to live independently in their own homes for as long as possible.

 

The post holder will work both as part of the general practice team and a wider multi-disciplinary ICC team, playing a role in coordinating care and support to individuals.

 

The post of CNM 2 has a pivotal role in service planning, co-ordinating, and managing activity and resources within the clinical area. The main responsibilities are: quality assurance, resource management, staffing and staff development, practice development, facilitating communication and professional / clinical leadership

 

There is currently 1 permanent whole-time vacancy available in Cavan & Monaghan Hospital. The successful candidate will be primarily based in the Emergency Department in Cavan Hospital.

Essential Criteria

1.  Statutory Registration, Professional Qualifications, Experience, etc

(a) Eligible applicants will be those who on the closing date for the competition:

 

(i) Are registered in the relevant division of the Register of Nurses & Midwives maintained by the Nursing and Midwifery Board of Ireland [NMBI] (Bord Altranais agus Cnáimhseachais na hÉireann) or entitled to be so registered.     

AND

(ii) Have at least 5 years post registration experience (or an aggregate of 5 years full-time post registration experience) of which 2 years must be in the speciality or related area of gerontology.

AND

(iii) Have the clinical, managerial and administrative capacity to properly discharge the function of the role.

AND

(iv) Candidates must demonstrate evidence of continuous professional development.

AND

(v) Candidates must possess the requisite knowledge and ability including a high standard of suitability and clinical, managerial and administrative capacity to properly discharge the functions of the role.

2.  Annual registration

(i)  On appointment, practitioners must maintain live annual registration on the relevant division of the Register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann).

AND

(ii) Confirm annual registration with NMBI to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC).

  

3.  Health

A candidate for and any person holding the office must be fully competent and capable of undertaking the duties attached to the office and be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service.

 

4.  Character

Each candidate for and any person holding the office must be of good character.

 

Post Specific Requirements

  • Demonstrate depth and breadth of experience in the area of gerontology nursing as relevant to the role.
  • Have undertaken or be willing to complete a post registration programme of study and achieve a Quality and Qualifications Ireland (QQI), National Framework of Qualifications (NFQ) major academic Level 8 award relevant to the specialist area of Gerontology as soon as this education is available post recruitment.
  • Access to transport as the role may involve travel to other hospitals in the region and a flexible approach to working hours is required.
  • This role will be 5/7 and a flexible approach to working hours is required. The role may develop to require a 7/7 working week in the future depending on service needs.
